How do I know if my loved one needs home care?

What is the difference between home care and home health care?
Home health care is medical care delivered at home by licensed nurses or therapists, including wound care, IV therapy, and physical therapy. It is typically ordered by a physician. Home care, which is what Nurturing Care Partners provides, focuses on non-medical support: personal care, companionship, household help, and daily living assistance. Many families use both in combination, particularly after a hospitalization.
How quickly can care begin?
In most cases, care can begin within 48 to 72 hours of the initial consultation and in-home assessment. Can you support someone with dementia?
Yes. Our caregivers receive ongoing training in dementia and memory care techniques including routine-based engagement, gentle redirection, and patient, calm communication. Consistent, familiar companionship is one of the most effective approaches for supporting individuals with dementia, and we take the caregiver-client match particularly seriously in these cases.
What makes your nutrition support different from other agencies?

How are caregivers screened and selected?
Every caregiver we place completes a rigorous multi-step process: a comprehensive national background check, verified personal and professional references, an in-depth interview, skills assessment, and ongoing training. We do not take shortcuts in this process. The safety and wellbeing of your loved one depends on who walks through their door, and we hold that responsibility seriously.
How do you match a caregiver to my loved one?
Caregiver matching is one of the most important things we do. We match based on required skills, relevant experience, communication style, and personal compatibility with your loved one's personality and preferences. We do not send whoever is available. We send the right person, and we take the time that requires.
Will the same caregiver come each visit?
Yes. Consistency is fundamental to how we operate. We protect the caregiver-client relationship as a core part of care delivery. The same caregiver follows the same care plan on each visit, building the familiarity and trust that genuine care depends on. If circumstances require a change, we communicate transparently and manage the transition with care.
What if my loved one does not connect with their caregiver?
We rematch without hesitation. The right connection is the foundation of effective care, and we take that seriously. If at any point the match does not feel right for any reason, contact your care coordinator and we will find a better fit promptly. Your family's comfort is the priority, not defending an initial decision.
Are your caregivers insured and bonded?

Care Plans
What is involved in building a care plan?
Our care plans begin with a thorough in-home assessment conducted by our care coordinator. We observe the home environment, spend time with your loved one, and gather detailed information about daily routines, personal preferences, physical and cognitive needs, safety considerations, and family expectations. The plan is then built entirely around those findings, not adapted from a template.
Can the care plan change over time?
Yes, and it should. A care plan that does not change is a care plan that has stopped paying attention. Our coordinators review and adapt the plan regularly based on caregiver observations, family feedback, and changes in your loved one's condition. You can also request a plan review at any time by contacting your care coordinator directly.
Can family members be involved in building the care plan?
Absolutely. Family involvement is an essential part of our planning process. You know your loved one better than anyone, and the care plan reflects that knowledge. We also welcome input from your loved one's healthcare providers, and actively coordinate with discharge planners and case managers when relevant.
Is there a cost for the care plan or in-home assessment?
The initial consultation and in-home assessment are provided at no charge and with no obligation. The care plan is built as part of beginning services with us. There is no separate fee for the planning process, and no hidden costs associated with plan reviews or updates.

What payment options do you accept?

Can I manage care from out of state?

Distance does not have to mean uncertainty.
What if I need to change or pause care?

We respond to all referrals within 24 hours and can begin care within 48 hours of the completed home assessment. For urgent discharge situations, call us directly and we will prioritize your case. The critical window after discharge does not wait, and we do not either.
Will you communicate updates back to the referring team?
Yes. We communicate proactively with referring teams throughout the engagement. You are never left wondering about a patient you referred to us. We share care updates, flag concerns promptly, and remain a reliable point of contact for the clinical team throughout the patient's home care.
